The problem

A referral letter drafted with AI adds a family history nobody asked about, strengthens a diagnostic phrase you never wrote, or carries over the previous patient's diagnosis from the same chat. An invented dose or study can enter the chart as evidence.

A process from your work

How long does it take you to write up your visit notes?

  1. Step 1With the patient's consent, the tool listens to the visit and drafts the note for the record.
  2. Step 2You read the note before saving it and correct any line about something that did not happen in the visit.
  3. Step 3The diagnosis, the decision and the signature stay yours.
  4. Where the tool goes wrongThe note adds a lung examination and rules out chest pain, though neither happened.
  5. The verification stepBefore saving, delete every finding for an examination you did not do today.

More: what was measured and the full check

What changed, as measured

In the randomised trial, time spent on notes fell by 0.36 hours a day, about 21.9 minutes, for those using the AI scribe.

Who was measured: Doctors and other practitioners in outpatient clinics of one US university health system, randomised to use an ambient AI scribe or to work as before.

The caveat: Participants volunteered, so they may be early adopters of new technology; the study was run in a single health system.

The check, step by step

  1. Before saving the note, read only the examination section and ask of each line: "Did I do this examination today?" Delete any finding for an examination you did not do.
  2. For every "denies" or "no" in the note, recall the question you asked the patient. If the question was not asked, the line is deleted.
  3. Write down yourself what was not examined, for example "Auscultation not performed", so the section is not left empty for a normal finding to fill.
  4. If the tool uses a template with an examination section, run a test consultation with no examination at all and see whether the tool still fills that section.

The course covers notes, letters and messages drafted with AI, with every fact checked.

A documented failure pattern

“Chest auscultation: no added sounds”, without a stethoscope

A family doctor at a health centre uses an AI tool that listens to the consultation and drafts the note for the patient's record. The doctor reads the note and saves it.

Why it is easy to miss

The doctor who opens the record next week will read that the lungs were examined and chest pain was ruled out, although neither happened. The two added lines are the ones almost every routine note contains, an examination finding and a denial, and they are normal findings, so they raise no alarm. The first line matches what the patient said, so the doctor recognises their own consultation and reads faster. In a 2025 study of five such tools, one wrote that a patient was alert when lethargy had been observed on examination. In a hospital trial in Australia (2026), 47% of staff reported added clinical information that had not been discussed, and the authors linked some of it to customised note templates.

Where the failure pattern is documented: Peer-reviewed studies: Mayo Clinic Proceedings: Digital Health (October 2025), BMC Health Services Research (Gold Coast Hospital and Health Service, January 2026), JMIR Medical Informatics (July 2026); academic study of automatic speech transcription, ACM FAccT (2024).Sources: Mayo Clinic Proceedings: Digital Health, 2025; BMC Health Services Research, 2026; JMIR Medical Informatics, 2026; arXiv preprint of an ACM Conference on Fairness, Accountability, and Transparency paper, 2024.The case above is ours, written for work in Albania; the laws it names were checked against the official text.

What it looks like

In the consultationThe patient said: "I've had a cough for five days, no fever." The throat was looked at. The lungs were not listened to with a stethoscope, and chest pain was not asked about.

The tool's noteCough for 5 days, no fever. Chest auscultation: no added sounds. Denies chest pain.
